EPIC Survivor Skydive Registration Form
I'm a cancer survivor and would like to be considered for the skydiving experience.
This is for a tandem skydive May 18, 2024 at Skydive Paraclete XP. Participant meets physical height, and weight requirements and be at least  18 years old. there is a $50 registration fee to secure your spot - sports are limited.
